Secure Forms Through NexHealth
After an appointment is booked, the office can assign the forms needed for that visit through NexHealth. Complete only the forms sent to you.
New Patients
Start by booking online through NexHealth. If you have already booked and cannot find your forms, call (303) 659-5185 so the team can resend the secure link.
Before Your Visit
Bring a photo ID, your insurance or Medicaid card if applicable, and a current medication list. Ask your previous dental office to send recent x-rays if you would like us to review them.
HIPAA Notice of Privacy Practices
The practice's current HIPAA Notice of Privacy Practices is available through NexHealth and from the front desk. Call the office if you would like a copy before your appointment.
